You must bring the following:
- Your insurance card(s) and some form of ID (social security card or photo ID)
- Physician referral forms if required by insurance
- A list of current prescription or over-the-counter medications you are taking, including the dose and frequency
- Pertinent information about your medical and surgical history. Bring any pertinent x-rays or records you may have.
Because our urologists are also surgeons, they sometimes run late. We will make every attempt to alert you if this occurs. Likewise, please inform our office as soon as possible if you cannot make your scheduled appointment.
